The FIQS research project gathers patient and family experiences to improve patient care and safety. Anything you share with us, good or bad, is helpful! We will use the information to help improve care.

What Went Well
YOU are an expert about you or your loved one's care AND an important part of the care team. We want to know what you experience so we can do better.
Safe patient care means care that keeps you/your child as healthy and well as possible while in the hospital.

We share comments with hospital safety staff BUT we do not share patient names unless you give your contact information when you write the comment.
The research team will also look at the comments and will track whether things change in the hospital in response to the comments.

Your comments are shared with hospital safety staff anonymously, without you or your child’s name, unless you give your contact information when you write the comment.
If you give your contact information, we will share it with hospital safety staff so that they can follow-up if they have questions.
If your comment describes a very specific situation, someone reading it might know who it is about even if you do not give your contact information.
If you don't want to participate, you can just text back "STOP" at any time. Participating in research is always your choice.
Please ask hospital staff to add them as a contact in the patient medical record.
