This study is testing Family Input for Quality and Safety (FIQS), a mobile-based survey tool that allows hospitalized children's families and adolescent patients to report safety concerns in real-time by responding to a text message link on their phone. By responding to a daily text message, you can report safety concerns, highlight what’s working well, or share anything you think could improve care. Your feedback helps care teams better understand patient and family experiences and supports ongoing efforts to improve safety and quality in the hospital.
